There is a growing demand for improved patient safety in healthcare from a variety of sources including patients, practitioners, administrators and regulators. Approximately, 44,000-98,000 patients die every year from errors. Patients in the intensive care unit are vulnerable to errors. The literature citing error associated with continuous renal replacement therapies is minimal and suggests that reporting rates are low. This session will review the concepts of systems based approaches and the application of error prevention to continuous renal replacement therapies.
